How widespread is RMON and RMON-II support in network devices?

I'm curious as to how widespread RMON and RMON-II support is inside network switches and routers.  Is this feature generally available, or is it something that few vendors support?

Is it a paid-for option (ie: not widely deployed), or is it free as part of the network device OS?

Please provide as much details and backup support (URLs if possible) to backup your answer.
