Exchange Server appears to be accepting outside requests to send spam

Our Exchange 2003 server looks to be spamming people.  I've run antivirus and antispyware scans on all servers including that one. I looked at the SMTP virtual server, which I did not set up, and anonymous access was enabled under the access tab.. I thought this might be the problem, but disabling it appears to stop ALL e-mail from going through the server..
