Attempt direct delivery before sending to smart host, does not work

Hi
I have a SMTP server on my web server.  I have a smart host that works fine and all mail sends.  If I tick box "Attempt direct delivery before sending to smart host" then some email fails, but the same mail works fine when not ticked.  At what point should it use smart host if you tick the above.
