SMTP Relay question

Hi

We have an application on a server that can send notification mail.

Within the application we can set an 'SMTP server' in the Notification Mail configuration. I guess this is either our Exchange server or an SMTP relay server.

What exactly does this mean? What does an SMTP relay server have that the application server doesn't, and how would an SMTP relay server send email to an Outlook client (via Exchange)? Is it not possible for the application server to send the mail directly to the recipients without having to enter an SMTP server?
