What causes SMB connection failed errors from Mac OSx 10.6.8 and Windows Server 2012

We have a very interesting issue occurring on a windows Server 2012 domain environment with one iMac running OSx 10.6.8.

In the past all connections from the Mac to the server have authenticated and were maintained relatively stable. No complaints from the user. Then, randomly one day the user called and said he wasn't able to see his "shared" drive anymore.

Every time we try to recreate a new connection it just reports the following error:  There was an error connecting to the server "X.X.X.X". Check the server name or IP address, and then try again.  If you are unable to resolve the problem contact your system administrator.

We performed all the basic troubleshooting steps:

1. Rebooted the Mac
2. Rebooted the Server
3. Check windows updates applied to the server to see if there was a file sharing or SMB related update applied right before the disconnect happened. (nothing directly related to these things were in the verbiage of the KB articles).
4. Connect to share on different communication protocols ie. SMB// AFP// CIFS//
5. Reviewed other machines on the domain to see if they were having issues connecting to the share. They are not.

Then we discovered an issue with one of the Toshiba all-in-ones where users weren't able to scan to a file (located on a server share on the same server the mac was trying to connect to). The last day that we had on record of the users scanning to file was December 4th 2014... interestingly enough there was a single windows security patch related OLE installed on the server on that date. (We do our audits on the 19th of the month for this customer so non of our techs would have installed an update manually on this date).

In any case we uninstalled the patch and rebooted the server... but still to no avail. We still are unable to get the Mac shared drive to get to connect and the toshiba will not scan to file; however, if we send the toshiba scans to a windows PC share on the network it works fine. So it seems the issue is isolated to SMB communication to the Windows 2012 server from both the toshiba and mac (PC network drives are fine). I can get the list of updates installed on the server if necessary but we are confident they were unrelated.

One additional "weird" thing we noticed was the only device appearing in the "shared" section of the sidebar on the mac was an epson camera. None of the other windows devices were showing up. There are about 4 or 5 other devices on the network.

I am not sure if the two issues are related or have nothing to do with one another but it just seems odd they both started occurring around the same time.

Any other ideas out there?

Possible cable, switch or router problem? Any possibility of an IP address conflict?

Self determined after  replacing device the settings worked again.
