You need few ports to be opened if you are using SIP services.
Usual Ports: SIP Server 5060, if your Softphone is using 5060 as its port then open it for TCP and UDP.
Next is media transfer ports used by RTP and UDP which is also mentioned in the softphone configuration.
Where are you checking the logs which says 408 "Request Time out". Are you talking about the logs shown by Softphone. If possible use Etherreal to monitor and read logs.

I am afraid I don't have an answer for you but the Avaya phones I have been involved with require a phenomenal number of ports to be open. The recommended method by Avaya, is to set up a VPN (Virtual Private Network) for any phone connections. With the VPN you have a secure encrypted connection without having to open/forward any ports, and not 'wide open' like you would be with a DMZ. Just a suggestion.
