The FE 0/0 is our LAN interface outside the firewall. I will hard code the duplex/speed.
As for the codec, we will be running g.711, but since we have only 5 trunks available to us, I arrived at 450k based on 90k per call. The SIP bandwidth may be overkill.
Any recommendations on which ISPs will shape traffic? The circuit was supposed to be installed yesterday, but it isn't, so I don't need too many more reasons to cancel it and go with another provider... ;)

The policy looks ok. A couple of things - FastEthernet 0/0 is the LAN interface or WAN? You should hard code the duplex to full 100, otherwise you can get clipping when the link negotiates. Also you only need the policy applied to the serial interface, remove it from the LAN.
What voice codec are you running? If g729 450kb should be ok, if g711 that may be a bit low as you need ~100kb per call. I usually reserve 1mb but it depends on your usage. If no voice calls are in progress the whole bandwidth is available for data.
The other thing to consider is XO will not shape on their end of the link ( i know from experience) so if you are using the full T1 you will experience QOS problems when the voice comes back in from their network.
