Cisco IP Phones, every other ring goes to voicemail on one extension

I have recently been thrust into the position of supporting my company's network and VOIP phone system.  We use a Cisco Unified 500 switch running several Cisco 7971 IP Phones.  Management is through the web-based interface, Cisco Unity Express version 3.0.  I did not set up the system.  Unfortunately, my experience is with Mitel digital PBX and very limited Mitel VoIP.
The problem I've been having has been giving me trouble for the last few weeks.  I have one extension that, when called directly, goes straight to voicemail EVERY OTHER CALL.  No rings, no missed call indicator, just voicemail.  The strange thing is that it is a member of a hunt group on a helpdesk number, and all helpdesk calls roll over to it just fine.  This only occurs when calling that extension directly, from both inside and outside numbers.  This is the only phone and only extension that is having this problem, everyone else is working just fine.
I went through both "Users" and "Extensions" settings.  I am fairly certain that all the settings are the same for the other extensions.  The only problem I can see is that under the Configure tab and Extensions, the extension is listed twice.  The first, sequence number 16, is a user that had been deleted from the Users section before my time.  The second, sequence number 17, is the current user and the one who I need to fix this for.  I've been told it is the same phone and the same extension that is being reused for the current person.  The phone and extension are both assigned to that person under the Users area.
Since this is obviously a duplicate, I tried to delete the old extension (sequence 16) and I get the error "Configuration change failed. Invalid ITS Command Option."  I am wondering if I need to change the sequence numbers first, but there is no readily available option to do so.
I would very much like to get this resolved.  Needless to say, this user is getting rather frustrated with me.

Unfortunately, I have now had reports that calling this extension from another internal extension goes directly to voicemail every single time.  With some checking, I discovered that it this only happens now with that one extension, the other phones still get the every-other treatment.
Please help!  I'm dying here.

Forgive me for the super late response.  I have been uber busy and working out of the office most days!  Yet still, this remains an issue.

Some background, Andrew (extension 5586) is the one with this problem.   Also, Brandon is no longer an employee.  Anything that can safely be deleted regarding that name should be.
Andrew, Mathew, and Nicole are the new employees.  Their extensions were set up using the web based manager and they seem to work.  Tony is an employee, but no longer works in this office.  I believe Andrew has Tony's old phone and extension, so there is likely some conflict with that.

Here is the *entire* output of show running-config.  I sincerely hope it helps.  I admit I am rather unsure what a lot of these are in here for, such a lot of the access control lists.  Enlightenment desired.

Once again, assistance is extraordinarily appreciated.  Please save me!

One thing that is immediately noticeable is the Ring No Answer field for 5586 is set to 10 seconds, this can cause issues with not being able to answer the phone in time and it going to voice mail.

Now a question though is if it is consistently every other call, have you been able to replicate this yourself, where you go straight to his voice mail, if so does your end ring at all or is it straight to voice mail for you with no ringing and is he on the phone when this happens and maybe he is not getting a call waiting tone? it could be a ring setting when his phone is active (i am not familiar with the options in this exact version) and have you actually seen that there is no missed call? I have noticed especially with mobile phones that i will get missed calls and it never rang, also sometimes it only does half a ring. This is more so to do with the roaming of the mobile phone but a question off of this, is there anything special about this user? is he in the same office with everyone else who is working fine? or is he in a area where the network is slower?

If all else fails and you feel comfortable re-programming everything I would delete the DN, phone, and user and re-create them. If Mathew and Nicole are working fine and Andrew is the only one with a problem, start from the beginning....

you are not using ephone-dn 16 so delete it and it should work.

How do I delete ephone-dn 16?  When I try, it says it is still a member of the hunt group.

delete the hunt group then delete the ephone-dn 16 after that recreate the hunt group.

Just out of curiosity, if I delete the hunt group will the customer call number go down until I recreate it?
The way I see it, this whole problem is caused by having two entries for the same extension.  I cannot delete one because they are both in the hunt group- a hunt group that is required for our business.  So obviously, I'm a bit nervous about blowing it away!
I have, of course, done a text-capture to back up anything I did before and after.  I am just worried about deleting something that works for all the phones besides this one...
