Administrator can set budget in ( money) per user, when the budget reached out, user will degrade of
class of service... and email send to user, manager , admin..
Manager and administrator can unlock for user.

answer is, there is no ready "off the shelf" application, because it's quite complex to develop to a good level of flexibility and customization. (It also involves administrator rights and screens, database for usage and credits, table toll computation, and probably too many things to make it practically viable.
I could use TCL/IVR documentation (for Cisco) to develop my own script or I can find someone to do it :)
