On a RJ-11 the middle two connections are one phone line and the outer two are the second. You are going to have to just stick the desired wire pairs in properly. It's not exactly ideal and looks a bit rough.
Using a wall jack with screw or punch down terminals might be better for a permanent setup as it looks a lot cleaner. Then just use typical phone cable from the jack to the device.

well - you wouln't normally crimp Cat5 wire to an RJ11 - you'd use an RJ-45 plug.
beyond that, are you looking for color standards (ie.. what color to what pin?)
or are you looking for what kind of tool to use, etc?
