RightFax Receive failure (Phase B).

How can I resolve a "Receive failure (Phase B)" error from RightFax 9.3 using a Brooktrout TR1034 family card?

I've checked my configuration against the support docs at captaris.com, but nothing seems to quite match my circumstances. One of the questions references a Cisco 6608 (answer id 117) which I dont' have - I'm connecting to an Inter-Tel 5400, and the other references a different family of fax boards (answer id 39).
