im attaching a very good troubleshooting doc from Cisco's site that lookss like it may help you. With the limited info I have so far , I think this doc might be your best bet to start anyway.

Browse All TopicsI have a Cisco AIR-AP1131AG-A-K9 wireless access point. This AP is an autonomous (stand alone) AP not used with a wireless controller. I used Cisco's upgrade tool & the correct image to upgrade the AP to LWAPP (so it could be used with a Cisco wireless controller). Now my controller does not recognize the AP I upgraded. So...now I want to downgrade or flash the Cisco AIR-LAP1131AG-A-K9 access point via hyperterminal & a console cable. Can anyone tell me what commands to type in to reset the whole image/AP back to factory default? To be honest with you, I don't know what kind of state the AP is in at this time after the image upgrade. It does get an IP address from my DHCP server but I can't telnet into it. ANy help will be appreciated.

With a console cable connected during bootup, do you get any output to your terminal ?
