I'm using EAP/PEAP/MS-CHAP-v2.

It turns out that the SSL Server Hello Certificate packet (this is what MS Network Monitor calls it) was not being accepted by the swich it was plugged into. Not quite sure why this was the case, as it had been working previously. Regardless, we updated the NIC drivers and firmware, and it started working again. Still not sure what caused the problem, we're guessing some property got changed somewhere and updating the driver cleared it out.

I should note that the access policy is linked to a group in AD. If i take the user out of that group, I do get failure entries in the event log and the IAS log, but with the user in the group, i get neither. I've done a packet trace via MS Network Monitor and I do see traffic between IAS and the WAP when the client attempts connection, so I know it is at least getting that far. I also see the event log "A LDAP connection with.... is established", so I know that IAS is talking to AD okay.
