Custom forms in MOSS

Hi all,

I would like to create a form for a custom list.  The list contains several columns, but the ones I am interested in are:

Name (this column contains a person's name from AD - 'Person or Group' datatype that allows the choosing of a single person)
First Name
Last Name
Email

What I would like to do in my custom form is If data is entered in the Name field, the First Name, Last Name  and email are filled in automatically.
