2008R2 BUGCODE_USB_DRIVER USBPORT.SYS USBOHCI.SYS BSOD
This server just started crashing with this error. Bluescreen view says its usbport.sys or usbohci.sys.
I updated the chipset drivers and it didnt help. Can we just replace the driver files with updated versions or are we going in the wrong direction?

I need the latest minidump files.
The dmp files you have uploaded seem to be of 2012
052112-33259-01
052112- 05 stands for month 21 day and 12 for year.
I use windows debugger.

Not much info in the dmp file.
You can enable verifier and upload the latest dmp or install latest bios update to resolve the issue.
Enable driver verifier
1) Open an elevated command prompt
2) Type "verifier /standard /all" (no quotes)
3) Reboot your machine
4) Use machine again until it crashes
After the crash & reboot, go into safe mode with networking or normal mode.
Disable driver verifier
1) Open an elevated command prompt
2) Type "verifier /reset" (no quotes)
3) Reboot your machine
Upload the new dmp file

I turned on verifier and it ran for 6 hours with no crash. It seems to crash once 20 hours or so.
I will let it run, update the bios, and see what we have.

You can disable the driver verifer.
Again ..not much info in the dmp file.
1. Run test on memory and hard drive. Can be a failing raid controller.Install latest drivers for raid. (MEGASAS RAID Controller Driver )
2. Can be smps issue. Check whether RAM is getting enough power.
3. Make sure overclocking is disabled in bios.
4. Check the driver version for usb devices in device manager.
Last option is clean install.

How do we know if the usb drivers are out of date?
We installed the latest chipset drivers from intel. This is an S1200BT mainboard.

None are yellow, and I can check the date. But a data of 12.9.12 doesnt mean it is out of date.
Besides installing the MB drivers from intel, how do you know if there is a newer USB driver?
We already disabled verifier.
If we disable the onboard USB all together, will it cause a problem? We can also installed a PCI USb card if we need to.

Accepted answer: 0 points for kassant7's comment #a39541077
for the following reason:
Driver was out of date. The intel installer did not update the driver so we had to do it manually.
