You can try run a array diagnostic provided by the server manufacturer i.e HP, IBM or Dell and form there you will know the root cause of the problem. Sometimes, a firmware upgrade can help you to fix the problem but a faulty HDD which attached to the array controller still need to be replaced.

Many times the drive array controller is located on the controller card itself, or on the chassis' mainboard. I would suggest getting the technical docs for the unit and looking there.
What type of device is in question?
