I ran into that problem once while using an iso with vista.
I started over again by recreating the vm, mounting the iso in hyper-v manager on the machine, making sure legacy network adapters was checked and basically setting up the entire thing in the management console.
I had the iso on my OS partition. I ripped a new iso from the Vista disc and placed it on a storage partition of the server i was installing the VM on which was the same partition i was storing the vhd's on as well.(once i clustered the machines I stored it on a NAS and didnt have the problem, yes you can use a nas and not a san when using a failover cluster and hyper-v)
After that the problem didnt happen again.
I can not tell you if it was the new image or the partition i stored it on, but that is what worked for me.
