My first response would be to create a survey. THis is done by going to the main page and clicking on view all site content -> Create ->Survey. You will then be able to set up the questions and settings. One thing to remember is that everyone you want to fill out the serveyu must have contribute rights to the survey or they will not be able to save thier choices. If you are not looking for a survey explain to me what you mean and I will see what I can think of.
