There's no difference. If you have hosted Exchange mailboxes, you will get the same service as you would get from an internally hosted Exchange Server.
The benefit of a hosted solution is that it allows you to concentrate on other matters and not worry about administering Exchange or some other mail server; the downside is that all the traffic is going out over the Internet to the mail server and you cannot manage all of the settings on the server.
