Icons get all messed up!

I am running NT (both server and workstation) with IE4 and some of the icons keep getting screwed up. It kind of looks like icons get superimposed on top of each other. For example, a folder icon will have the network drive icon stuck *over* it.

The only way I can fix it is to use TweakUI and "rebuild" the icons. However, this is only temporary since if the user logs off and then back on, the icons are screwed up again.

Any ideas how to fix this? I downlaoded and installed the latest version of IE4 and the problem seemed to go away but it can back.
