Add Program to Start Menu

First,  this question is not about creating a simple icon or placing an Icon in the Upper Left side of the Start Menu.  This is custom icons.
For instance, when you receive a server or desktop from a vendor (i.e Dell or HP), sometimes on the Start Menu they have created an special icon and it is placed under a divider on the lower right side of the start menu above the Log Off and Shut Down buttons.  How do you create one of those types of icons in the Start Menu. For instance, in the attached doc file,  you will see the Windows Security Icon above the Log Off and Shurt Down Icons. That is what I want to create.
