Sounds like it could be a RAM issue. Try reseating the RAM or only take it all out but one stick at a time until you discover the faulty module.

I am having HP PROLIANT BL46O G6 Server with Windows 2003 Server.
Now My Server is restarting frequently & after restart it is unable to connect to DB Server. Below are the 2 errors I am getting in my Event Viewer System Logs.

Run that program (bootable CD) and make sure you don't have some bad sticks of RAM

To paraphrase:
Faultrep DLL is a dynamic link library file for xp and 2003 server that reports faults. Since this is the faulty application, it could be one of three things:
1) a corrupt file
2) an incorrect version
3) Non registered DLL.
You have four options to help you out. None of these options are very invasive procedures.
First, do a system search for Fault.DLL and find out its location.
1) To overcome a corrupt file, you can download the appropriate version and then re-register it. There really is not telling what the correct version is.
2) System File Checker will check to see if you have the appropriate versions of important files on your system. If you have the incorrect version for your current OS, this could cause this problem. It also scans these same files for corrupt files. I am not sure if Faultrep.dll is one of the system files that SFC will scan for.
To use system file checker, go to the command prompt and type. ""SFC /scannow"
**NOTE: have your install disk handy. If a file is missing or corrupt.
3) Your third option is to try to re-register this DLL. To do so, go to the command prompt or a run line and type>
""Regvers32 faultrep.dll"
