The following things went wrong all at once:  Add/Remove Programs has disappeared from the Control Panel (double clicking "appwiz" does not restore it), Com Ports 3 and 4 have disappeared and been replaced by a general Com Port which is causing a hardware conflict and which, after I remove it, gets automatically restored on Windows startup as a newly found piece of hardware.  Also, when I attempt to use "Tweak UI" to repair system files, I get the set-up Wizard for Uninstaller 4 demo, which windows does not let me delete.  I have never played with the registry.  Good Luck.   Windows 95 came pre-installed on my PC, to which only I have access.  I have been playing with downloaded files-(is this a virus?).   At any rate, I have no idea how to re-install Windows 95.   My PC is from Packard Bell- is that the brand you"re talking about?  I fo have a master CD somewhere but since I don't know what I'm doing I'd like to take it long and bitter if that's possible.

someone did some major playing with your system.

do you want it short and sweet? delete all modems with device manager.
reinstall 95 using replace missing and corrupt files if that option is given.

what brand is the system?

Is there another approach. Yes. With PB format the hard drive and start over and dont put any of their junk back on your system. Unfortunately you would also have to buy a real copy of 95 because the one they gave you doesnt have a setup file on it. I went thru many problems with my nephews system and that is what we ended up doing.
Oh well.

first run scandisk using the thorough option. it may take an hour or more. use the settings free lost files and delete cross linked files under the advanced tab. see if tweakui works at that point.

go into device manager and delete the modem(s) you now have.
the phantom modem is supposed to be common with 95 but I've only seen it on PB machines. The solution is basically no solution. reboot your system and 95 should detect and reinstall a modem. if it does and it works you are ahead of the game. dont be surprised if the next time you start your system that the phantom comes back. if everything still works just ignore it. It may eventually go away on its own. The modem should be on com2 so make sure it is with device manager. the mouse should be on com1. make sure you only have 1 mouse installed also.

the missing icon could be a little tickier. if you find the file appwiz.dll you may be able to dblclick and reinstall it. the file should be in your windows\system directory. if this doesnt work I know no other way than to reinstall.

tweakui seems to have a screwed up association somewhere. if you still have your downloaded copy you can reinstall that and possibly restore correct settings. If not write down the name and location of the file that comes up. ex. c:\uninstall demo.exe.
got to a dos prompt in 95 (use start\programs) and type the following:
del c:\uninstall.exe
do not use the name I did. that is only an example. use the actual file name that you wrote down. if that gets rid of the file run tweakui again to see if it works. you may still have to reinstall it.
I hope at least some of this works but I am never real hopeful with PB.
