Make a share and map it to a drive letter.
e.g. share the folder c:\downloads\shareware as the sharename "sware". Then in explorer click Tools, choose Map Network Drive... select drive letter S from the picklist and enter \\computername\sware in the Folder field (where computername = the 'full computer name' entered on the Computer Name tab in sysdm.cpl). Check the Reconnect at Logon box and click OK. If C:\Downloads\Shareware is on a different computer, sometimes it works better to map the drive like \\192.168.1.11\sware (assuming the other computer's address is 192.168.1.11).
Then the drive letter S will show up in the drop down list just like C: and M: do above when you click on the My Computer icon in the common dialog places bar. Note that some programs use their own Save As routines instead of using the common dialog DLLs that microsoft provides, in which case you won't have the places bar on the left side of that dialog. But the drive letter : share remap will still be valid in that situation.

1) You can very simply customize the Places bar using TweakUI for XP. However five are the maximum.

2) You can add more using the Registry

I do not know if either works for Win7.
Perhaps of interest for Win7
