If you are accessing your computer from your home network on your laptop, you can get to it fine, because you are within a private network.
If you are accessing it from the outside, it is a different kind of connection. You will need to open a port on your router or firewall (port #3389) and connect to your home computer with its public IP address (you can find this in your router config page.)
The other option is buying a firewall with VPN access (Linksys has one for about $90) and setting that up, which is more secure than opening a port.
