Just for fun I tried this myself, creating a form I called "8x8" to make it unique. I have three profiles of one printer set up in my Printers & Faxes Control Panel; they have different defaults so I can quickly change from printing text on plain paper to printing photos on glossy paper without all that mucking about in Preferences.
I found that the new "8x8" form appeared in the default printer profile that I use for text, but it did not appear in the modified set that was limited to various sizes of standard photo paper. I could get it to appear by clicking on the defaults (aka reset) button in the Printer Preferences, along with all the other sizes that had been left out when the printer was defined as a photo printer.
Just to ensure I wasn't missing something by not using an Epson LX-300, I loaded the driver, and when I checked, my "8x8" form was available, right at the top of the list above "A4". And it stayed there, no matter what I did to other parameters of source and orientation!
It seems that yedunpaloor is right when he suggests you may be defining a paper size not supported by your printer. Are you defining a size wider than the carriage, or perhaps attempting to create a "banner" form. What size are you trying to create?

I dont think you can add a custom paper size to particular printer.

if you define a paper size thats not supported by your printer..
