Thanks...turned out to be a corrupted mail profile that needed to be recreated. A reboot was required instead of just an outlook restart.

Ran into a problem where a user in Outlook 2003 had a contact list that showed up on the left bar jsut fine. However, when they went to send an email, they would not have a contact list available to them when clicking on "To" in the email itself.
I ran through the fix. The same fix that is outlined here:

All is great now, but we have two contact list entries listed under "Show Names From The" pulldown menu. One contact is blank and the other is legit. I have seen this before and ran the fix that i knew of which is also outlined here:

Even if i remove all the entries including the main Outlook Address Book entry, the lists all still remain under "Show Names From the". I wasn't sure that is even possible since the user is running Oulook for POP mail. No global address book would be there. However, for some reason......all the lists ( 2 Contact List, Personal address book) still remain.
Any ideas or where to start?

just go to Control Panel and remove that profile.
Readd the profile with the same configurations as before but only add the data file, Outlook Address Book.
Open Outlook and open your Address Book
Select Tools >> Options
Make sure that "keep addresses in" has "Contacts" selected - this only becomes available with Outlook Address Book installed
Click OK
Close out of the Address Book and exit out of Outlook. Wait for Outlook to completely close (monitor processes in Task Manager if you have to) and reopen the program
From here, you should have your 1 contact entry which contains all of your "Contacts" from your Folder List
