Cleaning computers - Safe Mode vs Normal vs Boot Disk?

Bonehead question --

First -- I may be confusing "cleaning a computer" and "protecting it". I know that AV progs both scan for viruses already on the hard drive (cleaning), and also protect against incoming infection (emails, links, etc). I also know that some products like ccleaner or spybot are used for "cleaning" or scanning only, and don't provide "active" / "real-time" protection. Is there more of a distinction that I should know?

Second -- Since Normal Mode is very frequenlty too slow to run scans with, I usually opt to using safe mode. Someone suggested this is naive. But it usually works for me, If I can install and run AV in safe mode. What's the most common way to do it? How do you avoid Normal Mode when infected? The only other option I've come across is boot disks, like Hirens, and usnig the antivirus scanners on them. But the few times I've used one of those, it took even longer than infected normal mode. Maybe due to old PCs. (didn't try more than 3 times)

So my questions are -- What environment (normal,safe,dos boot) do you use for "cleaning", and does "cleaning" mean something other than running anti-virus/anti-badware scans?
