What is causing a Fatal System Error

I had two machine ona netowrk of 40 PC go down with continuous reboot cycle and then the server go down with following error:

STOP: 0xC000021A {Fatal System Error}
The Windows Logon Process system process terminated unexpectedly
with a state of 0x00000080 (0x00000000, 0x00000000).
The system has been shut down.


We brought server back by doing an OS repair. Now we are very concerned the server may fallover again without knowing when.
I would like to know (and need to answer to higher powers) why this happened. The only common factors between all machines are MS updates (recent VML patch) and McAfee Total Protection Solution.
If anyone is experiencing similar issues or has knowledge of causes please let me know

Hi Bartley1969,
I would look to the specifics of those two machines.
We use the McAfee product on all workstations and laptops and run all Critical Patches when they are published - and have not seen this.

Look for recent Warning or Error messages in the Event Viewer of those two.

Check also for any 'user' installed devices or applications.

I frequently find residue of unauthorized programs in the registry, after the user tries to hide what they did.

It turns out that McAfee are aware of this issue. They now have a fix - please see extract form McAfee support email below:
"as per discussion earlier the problem seems to be with the version of 'McAfee Total Protection for Small Business' you have.
On the server the System crash (Blue Screen) after installation of Total Protection for Small Business with a c000021a error and on the XP client machines they tend to continously reboot when the TOPs client has been installed.
As was mentioned, both faults can be addressed by modifying mfetdik.sys. The exact cause of this issue is currently under investigation.  It is believed to be an issue with the driver mfetdik.sys, which is installed with Total Protection for Small Business but not currently required for the software to function properly.
McAfee recommends the following workaround:
 
Disable the mfetdik.sys driver by renaming it to mfetdik.old.
Use Windows Explorer to navigate to C:\Windows\System32\Drivers.
Right-click mfetdik.sys and select Rename.
Rename the file to mfetdik.old and press Enter.
Restart the system."
