On a hacked mail server you would generally see a lot of mail queued up to go out. Type "mailq" to see what is sitting in the queue.
You can also install a package called pfqueue which can let you examine the queues in more detail.
If the queue has mail in it that is not from(or to) your internal users then its a possibility you have a problem.
make sure that you don't have a relay problem.
