Matrices (Need Expert Help)

Hi.Im just trying to make my own collision but im stuck with those matrices.I cant find where the coordinates are ??::

| A0 A4 A8 A12  |
| A1 A5 A9 A13  |
| A2 A6 A10 A14 |
| A3 A7 A11 A15 |

I this case what is x-axis ,y- axis , z-axis , position vector ???
I want to find current position of an object loaded from file after several translations and rotations.I think that one way (dont know if there is other) is to just find the coordinates on the new (rotated/translated) matrix and add to the real coordinates of my object. Example:

.                  x y z
We have a point T {1,2,3} and momental matrix state is
x-axis 1,0,0
y-axis 0,1,0
z-axis 0,0,1
center 0,0,0
I think this is clear  so far
well if we translate the matrix we would have:

x-axis 1,0,0
y-axis 0,1,0
z-axis 0,0,1
center 0,0,-5 (the translation is on z axis for -5)

the new point (T) coordinates would be:
Original T is {1,2,3} and new (T) is ( 1+0,2+0,3+(-5) ) And i dont claim that this is true but i think so (tell me if its not)

So After doing some rotations i dont know how to find the basic vector (dont know how its called on English but its the one that usually have magnitude 1 Example x-axis vector 1,0,0 - magnitude sqrt(1*1)=&& 1 && )I need this to calculate the current position of that point (object) so i can make my own collision (line collision).
