I have a high res model of human teeth which I wish to have a set of tools for dentists to drop/drag and draw on.
I imagine I will have to implement something along the lines of a UV unwrap program but I have no idea how to get started or if this is feasible in directX.

a ray-collission-detection feature should do this for you.
this is not very difficult since you do it in directx.
there is a sample from ms, called "pick" or so ;-)
it shows how to create the ray and do the collission detection with the d3dx-function D3DXIntersect which also returns baricentric uv-coordinates
