I checked the devices & all the activation dates are good (most several months before the last time this issue happened)
I will update the BES & test later this evening.
All BES services (including the above mentioned service) are started & set to Automatic
Wiping & reactivating the devices seems to have no effect on this problem at all, but I will also check that again after the update to SP6-MR5.

Hi, the Delete On option isn't set by the BES and it is only able to be changed on the device. If there is a corruption within the BES database the users device may automatically activate again (go to Options > Advanced Options > Enterprise Activation > Check the activation date). During this activation it will restore the backup settings from the device which obviously aren't being updated. Assuming you have BlackBerry Enterprise (i.e. not BlackBerry Professional) ensure you install SP6 and then MR5. Also start the BlackBerry Database Consistency service if it isnt already started. If this error occurs again after the updates delete the users from the BES, wipe the handheld and reactivate.
