I found some instructions on Putting the merge modules into a setup program, but that does not help on Click once deployment. Any ideas?

Right click on your project on the solution Explorer, select "Property", then select Prerequisites in Publish section, add “Crystal Report for .Net Framework 2.0”. If you are using report viewer, you need also select “Microsoft Visual Studio 2005 Report Viewer”.
