What is the most efficient approach towards global application settings and user settings

Hi all,

What is the most effiecient approach for the implementation of global application settings ( such as Application name, base dir , db connection strings  etc. ) ?
- web.config with reading those in run time
- resource file + Singleton class to load once during the start of the application
- else ?!

What is the most efficient approach for implementing user settings ( culture , display settings , db connection strings , specifig order of elements in GUI etc.
- settings table in db , retrieved and stored in a session obj ( some kind of collection .. )
- settings table , each setting retrieved only on demand ( e.g. when a page needing this settinngs is requested )
- else ?!
