Forms add, search and reset button

1. NetBeans IDE 6.5.1
2. Mozilla web broser
3. Apache Tomcat 6.0.18

Scripts:
1. JSP
2. JavaScript

Framework:
Hibernate

Problem:
There are 3 forms and they are the addMasterTreatment.jsp, searchMasterTreatment.jsp and the listMasterTreatment.jsp. In the searchMasterTreatment.jsp and listMasterTreatment.jsp form there is 3 buttons. The buttons are called search treatment, reset and add treatment. The search treatment functions well. However the reset and add treatment button does not function well. The add treatment button does not take me to the addMasterTreatment.jsp page. The reset button on the other hand keeps refreshing the page instead of reset the textfield control. This is happening when im using the mozilla web browser. Did I miss something?.
