Jeez it isn't half complicated. If i do this, will a MAPI call open a new email window using the new default form?

Choosing a custom user template for a new MAPI Outlook email
I have a Delphi app that sends attachments by MAPI. The customers will use Outlook. I am successfully sending the files and email at present but the customer requires the email to be sent with a Yes/No vote field to be selected by the recipient and returned.
I have a custom email template that does this, but how do I make OL open this custom form instead of the normal default one when I make the MAPI call?
I'm hoping it is just a matter of configuring OL to use the custom form by default anyway, but I can't figure out how to do that either!
