Adding items from Multi-Column Listbox to Table

I have a listbox with 3 columns.
I have a textbox.

I have tblTicketReason with 4 elements:

-ID
-FormID
-MajorReasonID
-MinorReasonID

I want to add the textbox value to the ID fiield.  And loop through the listbox columns and add them to the three other elements in the table.  So ID remains constant, and the listbox items fall into FormID, MajorReasonID, and MinorReasonID...
