Insert into table using Perl Script

I am on a really tight deadline and am really stuck by a problem I am having writing to a field I added to a mysql table.
when I trace the code I cannot see why it will not pick up the value from the web form. I have posted the code with the hopes that someone can help me today.

The user fills out the form online when hits submit. from submt a cig script is called which calls the insertShift, insert shift gets the values from the web form and inserts into the table. Problem is it does not get the value but gives me the error. I'm not sure where the error is all the know is that it will get get a value from the html form.
