JQuery range slider, how to pass values to form

I have the JQuery range slider on my web page but cant figure out how to pass the form values (both handles) to form fields. Cound someone please post a code example on how to accomplish this?  Any help is greatly appreciated!

Add two hidden fields in the form (the below code uses type=text to debug)
