This is your problem line

Its trying to reference a html element that does not exist. Could you post your HTML to see what does exist?

I would recommend doing an alert for all your "document.getElementById()" calls. For example:

alert("[ID_Of_Object]: " + document.getElementById('ID_Of_Object'));
alert("[ID_Of_Object2]: " + document.getElementById('ID_Of_Object2'));
.
.
.
alert("[ID_Of_ObjectN]: " + document.getElementById('ID_Of_ObjectN'));

When you get an alert with "null" as the value, that will tell you that object doesn't exist in the context. Then you need to make sure the you typed all your IDs correctly. Misspellings are common issues here.

I ripped out the the  which were referencing some sort of Dreamweaver template stuff. The physical code, I didn't need to touch just the wrappers. Seems to have done the trick for me at least though. Is it working for you now?

Nop still not working look at http://www.locumexpress.ie/formresults.html and i was using a template thats why they were there so if i have to get rid of them then i can use my template i add some text to your edited code to see what happens and the like above is the result

Found it! :)

There is a rogue <!-- within your style sheet that is canceling out certain elements (this is in the your link at http://www.locumexpress.ie/formresults.html)

Remove the <!-- that is on line 116 on your code you posted above.

N.B firefox rendered it fine, its only a IE bug
