Append so text to an existing file

I have a list of mac address in a flat text file. The mac addresses are the first coloumn in the file. I need some code to remove all other columns but the mac address AND add some text before and after the mac address.

for axample, my existing file data looks like this:

0015.a4ce.09c1 C5/0/2/U0 online(pt)     168   1.1  eCM eMTA    
0007.1102.6e72 C5/0/2/U0 offline        775   n/a  eCM eMTA        
0015.a4da.aa7d C5/0/3/U0 online(pt)     66    1.1  eCM eMTA    

I want to strip off the last 6 columns and then add the following text "before" and "after" the mac address.
"clear cable modem" goes before the mac address and "reset" goes after, so the fiisnished product will look like this:

clear cable modem  0015.a4ce.09c1 reset
clear cable modem  0007.1102.6e72 reset
clear cable modem  0015.a4da.aa7d reset

The script can eiother create a new txt file or edit the existing one, doenst matter to me.
