Rails application, changes to a database cause app to crash

I apologize for not being up to speed on the latest Ruby on Rails code and ways to develop, but need some clarification and understanding of an application.  The application is being designed by an outside firm and my challenge is to learn as much as I can about it.

We made a change to the database the Rails app is using by adding a table row, we did not modify anything else in the database.  After this change was made the Rails app stopped working with an Active Record error.  If we remove the row the app  begins to work again.

So I have a better understand - can you tell me what is causing this to happen?  Is the app scanning the database each time and will error out when something as simple as a row as been added to a table?
