Change form to modeless UserForms

G'day experts

I need someone to expand on a solution that is 90%  sorted for me.  If you could please take a look at this q

Basically the expert on that question has made me some code that contains a form to allow telephone messages to be sent quick and eaily.  What I want is the form to send, show confirmation that the mail was sent, reset the fields to empty and stay on the screen, the form should remain on top of all windows.  matthewspatrick from the other thread advises that I need to ask for a modeless UserForms

Appreciate anyones input in fixing this for me.
