Ok, I got the paneling down. My issue is when I select a radio button say for "Hamburgers", and when I check the various add-ons, how do I get it to add the subtotal, tax, and order total when I press the Order button?

OK, to start with your code does not seem to change the Panel Text "Add-on Items($.xx/each)" What i would suguest is having 3 of these panels and at any point of time any 2 of them have the .visible property set to false. There fore you dont need to change ANY text at all anywhere when a user clicks on any of your maincourse check boxes.
Each panel would give you 3 chkchoice buttons, so numbered 1 through 9 and you just allocate 1-3 to the first panel, 4-6 to the second panel and 7 through 9 to the last panel.
The rest then becomes easier. If your still struggling let me know and i'll boot up a VM with VB on it.
