I need a sample code or tutorial to do a multi column list box like what display in attached picture.

That's not a mutli-column List Box, that's a DataGrid control. Should come standard with VB6, to my knowledge.
Of course, you're better off switching to using VB.NET, for quite a few reasons, not the least of which is platform support and application efficiency.
