Basically you do the following:
- Make a form in VB(.NET) that has a box for inputing your parameter, or just pops up an input box for it.
- Take that parameter and use an ADO Command to run your query with it, and return a DataSet.
- Write the dataset out to a CSV file, using plain .WriteLine, or something fancy like opening a OleDBConnection with Text file source, then inserting the dataset into the table off that connection.
