SQL Query results to .csv file

I have a routing MSSQL query that I run and save the results to a .csv file.

I'm looking for an example of how to create a VB or .net app that I can create that will prompt for a date parameter, run the SQL query and then save to a .CSV file.

Any suggestions

Basically you do the following:

  1. Make a form in VB(.NET) that has a box for inputing your parameter, or just pops up an input box for it. 
  2. Take that parameter and use an ADO Command to run your query with it, and return a DataSet. 
  3. Write the dataset out to a CSV file, using plain .WriteLine, or something fancy like opening a OleDBConnection with Text file source, then inserting the dataset into the table off that connection. 
Do you need a full source?  I'll need to find a little time to write one then.

If you can write a full source example it would be appreciated.  I'm just getting back in to development and this would get me pointed in the right direction and with something that is relative to my current need.

The computer I'm using now doesn't have VS.NET on it.  I did an example with VBScript, since there's really only one visual interface - the box asking for parameter - so it's adequate just using script.  You can readily move the whole logic to VB/VB.NET without much fussing.  This is the very plain version: ADO classic (not .NET), and writing CSV manually with text stream.  Let me know if you need some more bells and whistles.  Not sure if I can test them on this computer, so it may take longer.
