Insecure: Open MSSQL port (1433/tcp by default) to the public (by forwarding it from the router). That leaves the port unprotected against attacks from Internet.
Secure: Create a VPN, and let them access the DB this way. The VPN will allow for secure (authenticated and encrypted) access.
