How do I write the code for a MojaveExternalPlaySeekMute.swf to show on a site.

I'm trying to distribute an embeding code to some sites, but the MojaveExternalPlaySeekMute.swf is not appearing.  

This is how the flash should appear:
http://thrilljockey.com/javelin/

But this is how it appears with the code I'm providing:
http://www.thrilljockey.com/flashads/

The code is below.  

I also, would like to have the player to start manually, not automatically.  Is this possible?
