How do I write the code for a MojaveExternalPlaySeekMute.swf to show on a site.

I'm trying to distribute an embeding code to some sites, but the MojaveExternalPlaySeekMute.swf is not appearing.  

This is how the flash should appear:
http://thrilljockey.com/javelin/

But this is how it appears with the code I'm providing:
http://www.thrilljockey.com/flashads/

The code is below.  

I also, would like to have the player to start manually, not automatically.  Is this possible?

Oh and you should really always use classid="clsid:D27CDB6E-AE6D-11cf-96B8-444553540000" codebase="http://download.macromedia.com/pub/shockwave/cabs/flash/swflash.cab#version=6,0,29,0"

themrrobert:  Thanks a lot.  I can't still have it to work.  Would it be helpful to mention that this code is meant to be posted on sites off my server?  This is the way I have the code that I'm supposed to share with other websites and  it still not working.
