The Solution Rating System

I have application running on a server that has centralised access, we keeping getting thsi "application error"  . This application is written in more in ASP and about 10% in ASP.net . On the server we keep seeing this happen, Please let me know how this can be debugged or any suggestions on how to find out the issue.
